-Types of cases required: General, Ortho, Urology, Opthalmology, Vascular, Dental, ENT, Podiatry, ***No OB
-Spinals, Blocks, Epidurals - must be proficient: yes
-Caselogs required: Yes - proof of epidurals, spinals and blocks
-Number of providers during the day: 4
-Number of ORs: 3
-EMR: EPIC
-Credentialing Timeframe: 60-90 days
-Licensure Requirements: ACLS, BLS, PALS
