ASSIGNMENT DETAILS: JO# 2823445
• Location: Near Sedona, AZ
• Start date: April
• Shift: (but be available for minimum 2 of the blocks below)
o April 21-25
o June 9-13
o July 7-11
o 1-2 weeks per month August forward
• Shift Details: Monday – Friday
o 7:00am-5:30pm – Cath Lab at the Hospital except for the below days:
o Every Wednesday 8:00am -5:00pm – In- Office Clinic
o Every 2nd Friday of the month 8:00am -5:00pm – Telemedicine Clinic
o Every 4th Friday of the month 8:00am -5:00pm – In-Office Clinic (First Patient at 9:00am)
o Night beeper call on Fridays
• Patients per day: Clinic 20-22 patients
• Patient Population/Age: adult
• Department Description: Cath Lab will be hospital consults 3-4 per day, plus EP cases 4-6 a day depending on case length.
• EMR: Cerner
• Hospital Bed size: 268
• Special Procedures: cardiac electrophysiology; specifically, heart rhythm disturbances, Ablations - must show 100 EP cases in last 2 years in case logs with emphasis on ablations
• Skills Required: Ablations
• Required: Board Certified or Board Eligible within 5 years of Residency, Active AZ license, DEA, ACLS, 1 year experience required
