Contract Length: Ongoing
Location: Arizona (Hospital & Outpatient Clinic Settings)

📅 Schedule Details:

Monday – Friday | 7:00 AM – 5:30 PM in Cath Lab (Hospital)

Wednesdays | 8:00 AM – 5:00 PM – In-Office Clinic

2nd Friday Monthly | 8:00 AM – 5:00 PM – Telemedicine Clinic

4th Friday Monthly | 8:00 AM – 5:00 PM – In-Office Clinic (Starts at 9:00 AM)

Night Beeper Call on Fridays

Confirmed Coverage Dates Needed:

June 9–13 | June 23–27

July 14–18 | August 4–8 | August 25–29

September 22–26 | Sept 29–Oct 3

October 13–17 | October 20–24

November 17–21 | Nov 24–26, 28

December 15–19 | December 22–26

➡️ Client prefers a provider with ongoing availability for at least 5 weeks of coverage.

🏥 Work Details:

Patient Load:

Clinic: ~20–22 adult patients/day

Cath Lab: 3–4 consults + 4–6 EP cases daily

EMR: Cerner

Hospital Bed Size: 268

Procedures: Must be experienced in cardiac electrophysiology, especially ablations

100 EP cases in the past 2 years required (documented in case logs)

Setting: Mix of Hospital + Clinic

Support Staff: Details TBD

✅ Requirements:

Board Certified/Board Eligible in Cardiology or Electrophysiology

Arizona License (or open to licensing)

DEA (AZ registered) & ACLS certification required

Minimum Experience: 1 year

Credentialing Timeframe: ~30 days
