• Home care visits
• 5–6 patient assessments per day
• 8–10 NP/PAs needed
• Must have a vehicle (travel within a 1-hour radius)
• Primary location: South Jersey, with some visits in North Jersey
• The client will provide iPads to providers for use during assessments
