APP Home Care Visits - New Jersey (more states coming: GA, NY, TX, VA, WI)

Job description

Job Details:
- Conduct 5–6 home health assessments per day
- Paid for a full day, regardless of patient volume
- Perform annual risk assessments and review of chronic conditions
- No treatment or prescribing – assessment-only role
- Escalate urgent findings to the patient’s PCP
- If applicable, conduct A1C tests only (other screenings left for the patient)
- Travel required; mileage reimbursed daily

Must be available between 8 am-8 pm, but will only work an 8-hour day.
Duration: 90-day assignment with option to extend

Qualifications:
- Must be Board-Certified FNP or ACNP
- Must be licensed in NJ
- Mileage will be reimbursed daily
- Will not cover travel outside of the daily mileage reimbursement, so local candidates are the best option for this job

Specific to the New Jersey location: Home visits across Ocean County (6 providers needed), Middlesex, and Morris County. Please specify your preferred county
