• Start Date: ASAP
• Shift Schedule: Full-Time (40 hours/week); Monday – Saturday (Flexible); 8-Hour Shifts between 8:00am – 8:00pm (e.g. 8:00am – 5:00pm or 12:00pm – 8:00pm)
• 5–6 home health assessments daily
• Paid for full day regardless of patient count
• Assessment-only role (no prescribing or treatment)
• Conduct annual risk assessments, review chronic conditions
• Escalate urgent issues to patient’s PCP
• May perform A1C tests (other screenings not required)
• Daily travel required; mileage reimbursed while on assignment
• Equipment provided
• Must be Board Certified
• Active NJ License
• Active NJ Medicare Number
