Job description

Facility in Toms River, NJ seeking NP locums, house call coveragae
Rates: $110/hr
Start Date: June 10th or June 24th, 2025
Assignment Duration: 90 days
Location: Home visits across Ocean County (6 providers needed), Middlesex County, and Morris County, NJ

Schedule:
40 hours/week, Monday through Saturday
8-hour shifts between 8 AM – 8 PM (e.g., 8–5, 12–8)
Includes a 1-hour lunch break
Schedules published 2–3 weeks in advance


Position Details:
Scope: Assessment only – no prescribing or treatment
Responsibilities:
Annual risk and chronic condition assessments
Escalate urgent clinical concerns to the patient’s PCP
Perform A1C testing when required (no other screenings)
Equipment Provided:
iPad
Blood pressure cuff
Lab kits
Qualifications:
Board-Certified Family Nurse Practitioner (FNP) or Adult-Gerontology Acute Care Nurse Practitioner (ACNP)
Active New Jersey RN/APN license
Active NJ Medicare number – mandatory
Comfortable with independent home-based patient assessments
Must be reliable with strong documentation skills
