• Start Date: July 2025
• Shift Schedule: Full-time, 40 hours/week (Monday–Saturday, flexible); 8-hour shifts between 8 AM – 8 PM (e.g., 8–5 or 12–8)
• 5–6 home health assessments/day
• Assessment only – no treatment or prescribing
• Review chronic conditions, perform annual risk assessments
• Escalate urgent findings to PCP
• May perform A1C tests (if needed)
• Daily travel required – mileage reimbursed
• Paid full day regardless of patient volume
• Equipment provided
• Must be Board Certified
• Active NJ License Required
