FNP needed in Bergen/Essex county

Job description

Monday-Friday 8 am - 5 pm or 9 am -6 pm

Job Details:
Conduct 5–6 home health assessments per day
Paid for a full day, regardless of patient volume
Perform annual risk assessments and review of chronic conditions
No treatment or prescribing – assessment-only role
Escalate urgent findings to the patient’s PCP
If applicable, conduct A1C tests only (other screenings left for the patient)
Travel required; mileage reimbursed daily

Credentialing:
1–2 week turnaround
Must have an active NJ license and NJ Medicare number

• 5-6 daily assessments
• Paid for a full day, regardless of patient volume
• Mileage reimbursed daily
