25% Outpatient Palliative Care


Position Overview

The Nurse Practitioner will deliver comprehensive inpatient and outpatient palliative care services, focusing on symptom management, chronic disease management, advanced care planning, and hospice transition discussions. The role requires independent clinical practice in a fast-paced hospital environment.


Key Responsibilities

Conduct patient consultations for palliative and hospice care needs.

Lead sensitive discussions with patients and families regarding terminal diagnoses and care goals.

Perform daily inpatient rounds, including ICU patients.

Manage outpatient palliative visits through the Oncology department, infusion center, Comprehensive Health Clinic, and Telehealth.

Collaborate with hospital specialists (ICU, Oncology, Pulmonology, Cardiology) to align care plans.

Provide education to staff and advocate for patient-centered care.

Maintain accurate documentation in Meditech (inpatient) and eClinicalWorks (outpatient).

Manage an average of up to 6 patient encounters per day over a 5-day workweek.

Requirements

Board Certified NP – REQUIRED

Ohio NP license (or willing to obtain) – REQUIRED

Experience as an advanced provider in an acute care setting or with palliative care/hospice – REQUIRED

Clean malpractice and licensure record – REQUIRED
