• Start Date: ASAP
• Shift Schedule: Must be Available between 8:00am – 8:00pm (Scheduled for an 8-hour shift within this window, e.g., 8 AM–5 PM or 12 PM–8 PM)
• Full-time, 40 hours/week, Monday through Saturday (flexible within that window)
• Conduct 5–6 home health assessments per day
• Paid for a full day, regardless of patient volume
• Perform annual risk assessments and review of chronic conditions
• No treatment or prescribing – assessment-only role
• Escalate urgent findings to the patient’s PCP
• If applicable, conduct A1C tests only (other screenings left for the patient)
• Equipment Provided
• Must be Board Certified
• Active NJ License Required
