• 6 month assignment
• Schedule: 8 am – 4:45 pm - Monday, Thursday, Friday.
• Full time or part time?: Both; prefer FT
• FM MD
• PPD 15-20
• Board Certified or Board Eligible? Board Certified
• Who will the providers report to? CMO
• How many NPs or PAs onsite?4 - NPs
• Type of support staff Ex: (RNS, MAs, etc.)? 2 or 3-MAs
• What’s your EMR System? Athena
• What is your Credentialing time frame: 2-3 weeks
• Are you Open to Travel and Lodging? Possible
• Are you Open to covering mileage? Possible
• Interview Process: Is it just one interview? Definitely will interview with the CMO and CEO (who is also a MD
