Home Health APP needed in New Jersey

Job description

Hello, this is Alisha, Senior Principal Recruiter at Aya Locums. I'm reaching out today about a new Home Health APP needed across Ocean, Middlesex, and Morris Counties. Please specify preferred county, if interested!

Contract Length: 90 days, ASAP start
Shift Details: Must be available between 8 AM – 8 PM (Scheduled for an 8-hour shift within this window, e.g., 8 AM–5 PM or 12 PM–8 PM)

Work Details:
Conduct 5–6 home health assessments per day
Paid for a full day, regardless of patient volume
Perform annual risk assessments and review of chronic conditions
No treatment or prescribing – assessment-only role
Escalate urgent findings to the patient’s PCP
If applicable, conduct A1C tests only (other screenings left for the patient)
Travel required; mileage reimbursed daily

Equipment:
iPad
Blood pressure cuff
Lab kits for patient use

Credentialing:
1–2 week turnaround
