Family Practice Home Visits NP/PA Needed in New Jersey
Start Date: June 2025 or as soon as credentialed
Contract Length: 3 months
Schedule: Must be available between 8 AM – 8 PM (Scheduled for an 8-hour shift within this window, e.g., 8 AM–5 PM or 12 PM–8 PM)
Full-time, 40 hours/week, Monday through Saturday (flexible within that window)
Schedules are released 2–3 weeks in advance
Job Details:
Conduct 5–6 home health assessments per day
Perform annual risk assessments and review of chronic conditions
No treatment or prescribing – assessment-only role
Escalate urgent findings to the patient’s PCP
If applicable, conduct A1C tests only (other screenings left for the patient)
Equipment:
iPad
Blood pressure cuff
Lab kits for patient use
Local providers preferred

Credentialing Timeframe: 1–2 week turnaround
Active NJ License
NJ Medicare number
