Home Health Assessments in New Jersey - New Grads Welcomed (NJ License Required)

Job description

A home-based care program in New Jersey is hiring multiple NPs and PAs for a 90-day locum assignment conducting in-home health assessments. Coverage is needed across Ocean, Middlesex, and Morris Counties—please indicate your preferred area when applying.

Assignment Details:
• Start Date: June 16, 2025 (or ASAP)
• Duration: 90 days
• Schedule: 40 hours/week, Monday–Saturday (flexible days)
• Patient Volume: 5–6 home visits per day
• Paid for a full day regardless of volume

Role Overview:
• Perform in-home health risk assessments
• Review chronic conditions and medical history
• Escalate urgent findings to the patient’s PCP
• No prescribing or treatment
• A1C testing only (no other screenings)
• Daily travel required – mileage reimbursed

What You’ll Need:
• Active New Jersey license and NJ Medicare number
• Comfort with independent home visits
• New grads are encouraged to apply!

Additional Info:
• Equipment provided (iPad, BP cuff, lab kits)
• Credentialing turnaround: 1–2 weeks
