Home Health NP/PA Needed in Northern New Jersey in Bergen, Hudson, and Essex counties New Jersey:
Start Date: July 2025 or as soon as credentialed
Contract Length: 3 months
Schedule: Monday-Friday 8 am - 5 pm or 9 am -6 pm. Full time is preferred but will consider part time.
Job Details:
All providers must be comfortable working across all three counties.
All three counties are near each other in North Jersey!
Conduct 5–6 home health assessments per day
Perform annual risk assessments and review of chronic conditions
No treatment or prescribing – assessment-only role
Escalate urgent findings to the patient’s PCP
If applicable, conduct A1C tests only (other screenings left for the patient)
Equipment:
iPad
Blood pressure cuff
Lab kits for patient use
Local providers - mileage reimbursed daily

Credentialing Timeframe: 1–2 week turnaround
Must have an active NJ license and NJ Medicare number
Medical malpractice insurance provided
