Locum Home Health NP - Northern New Jersey

Job description

Start Date: July 28th

Duration: 90-day assignment (confirmed by client)

Location: Home visits across Bergen, Hudson, and Essex Counties..

Scheduling:

Must be available to work 8 am- 5 pm or 9 am - 6 pm, 5 days per week, 40-hour work week

One-hour lunch break included

Schedules are released 2–3 weeks in advance


Job Details:

Conduct 5–6 home health assessments per day

Paid for a full day, regardless of patient volume

Perform annual risk assessments and review of chronic conditions

No treatment or prescribing – assessment-only role

Escalate urgent findings to the patient’s PCP

If applicable, conduct A1C tests only (other screenings left for the patient)

Travel required; mileage reimbursed daily


Equipment:

iPad

Blood pressure cuff

Lab kits for patient use


Credentialing:

1–2 week turnaround

Must have an active NJ license and NJ Medicare number


Qualifications:

Must be Board-Certified FNP or ACNP
Must be licensed in NJ
Mileage will be reimbursed daily
