Licensed Practical Nurse – Franklin, NH 03235 (#M1121)
Employment Type: Per-Diem (1 Day/Week)
Hourly Rate: $29.83 – $29.93/hr

✅ Qualifications
✔️ Completion of an accredited LPN program
✔️ Active New Hampshire LPN license
✔️ Current BLS certification
✔️ At least 1 year of LPN experience in rehab, long-term, or skilled nursing care
✔️ Skilled in wound care, med administration, and EHR documentation
✔️ Compassionate, detail-oriented, and an effective team communicator

✅ Key Responsibilities
✔️ Conduct routine assessments and monitor patient health
✔️ Administer medications and implement prescribed treatments
✔️ Perform wound care, catheter care, and assist with clinical procedures
✔️ Coordinate with physicians, therapists, and RNs for optimal care planning
✔️ Document patient progress accurately and report condition changes
✔️ Maintain a safe, clean, and therapeutic environment
