Position Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join a Skilled Nursing and Rehab facility in Albuquerque, NM. This per-diem position involves delivering high-quality care during 12-hour shifts, with a strong focus on rehabilitation, patient education, and teamwork.

Qualifications
✅ Graduate of an accredited LPN program
✅ Active New Mexico LPN license
✅ BLS and CPR certification
✅ At least 1 year of experience in rehab, long-term care, or acute settings (preferred)
✅ EMR proficiency and strong documentation skills
✅ Excellent communication, empathy, and teamwork

Key Responsibilities
✅ Administer medications, treatments, and wound care under RN supervision
✅ Monitor and document patient conditions and progress
✅ Support care plans with RNs, therapists, and physicians
✅ Educate patients and families about medications and recovery
✅ Maintain accurate and timely clinical records
✅ Follow infection control and facility safety protocols
✅ Provide emotional support and patient advocacy
