✅ Key Responsibilities
✔️ Administer medications, wound care, and treatments under the supervision of an RN.
✔️ Monitor patient health, record vital signs, and report clinical changes.
✔️ Support care plans and communicate with therapists, physicians, and families.
✔️ Educate patients and families on discharge care, medication routines, and wellness strategies.
✔️ Maintain detailed and accurate clinical documentation, medication logs, and shift notes.
✔️ Adhere to infection control and facility protocols while promoting a safe care environment.
✔️ Provide emotional support and advocate for each patient’s unique needs.

✅ Qualifications
✔️ Education: Graduate of an accredited LPN program.
✔️ Licensure & Certifications:
Active LPN license in New Mexico
Current BLS and CPR certification
✔️ Experience: Minimum 1 year in rehab, long-term care, or acute care preferred.
✔️ Clinical Skills: Proficiency in EMR systems, medication administration, and patient care planning.
✔️ Soft Skills: Detail-oriented, compassionate, and collaborative team player.
