✅ Qualifications
✔️ Graduate of an accredited LPN program
✔️ Active New York State LPN license
✔️ Proof of Hepatitis B immunity (required)
✔️ Minimum 1 year of long-term care experience
✔️ Excellent communication, reliability, and adaptability

✅ Key Responsibilities
✔️ Administer medications and treatments per physician orders
✔️ Monitor and document vital signs and patient conditions
✔️ Support residents with ADLs and assist with mobility
✔️ Collaborate with RNs, MDs, and therapy teams for optimal care plans
✔️ Follow infection control policies and facility procedures
✔️ Maintain thorough and accurate clinical documentation
✔️ Respond promptly to resident needs and emergencies
