Licensed Practical Nurse – Skilled Nursing | Jersey Shore, PA 17740 (#M1103)
Employment Type: Per-Diem (2 days/week)
Hourly Rate: $30.55/hr

✅ Qualifications
✔️ Graduate of an accredited LPN program.
✔️ Active Pennsylvania LPN license.
✔️ BLS and CPR certification required.
✔️ At least 1 year of experience in rehab, long-term care, or acute care settings preferred.
✔️ Must provide:
 - Compliance Attestation Form
 - Dementia Training
 - Influenza Vaccination
 - Active Professional License
 - Safe Resident Handling Training
 - Other state-specific documents
✔️ Proficient in EMR systems and accurate clinical documentation.
✔️ Compassionate team player with strong communication and attention to detail.

✅ Key Responsibilities
✔️ Administer meds, treatments, and wound care under RN supervision.
✔️ Record patient conditions, vitals, and responses to treatment.
✔️ Assist in executing care plans with the healthcare team.
✔️ Provide patient and family education on meds and post-discharge care.
✔️ Maintain thorough clinical records and shift reports.
✔️ Follow all infection control and facility safety protocols.
✔️ Offer emotional support and advocate for each patient’s well-being.
