✅ Key Responsibilities
✔️ Administer medications, treatments, and wound care under RN supervision
✔️ Monitor and document patient conditions, vitals, and progress
✔️ Collaborate with RNs, therapists, and physicians on care plans
✔️ Educate patients and families about post-care recovery and medication use
✔️ Maintain accurate clinical records and reports
✔️ Adhere to infection control and safety protocols
✔️ Provide emotional support and patient advocacy during rehabilitation

✅ What We’re Looking For
✔️ Graduate of an accredited LPN program
✔️ Active Pennsylvania LPN license
✔️ BLS and CPR certifications required
✔️ Minimum 1 year of experience in rehab, long-term care, or acute care settings
✔️ Strong EMR documentation skills
✔️ Excellent communication, empathy, and collaboration

✅ Schedule & Compensation
✔️ Per-Diem: 2 days/week
✔️ Shift: 3:00 PM – 11:30 PM
✔️ Rate: $27.56/hour
