✅ Key Responsibilities
✔️ Monitor and document patient vital signs and overall health status.
✔️ Administer medications, manage wound care, and perform catheterizations.
✔️ Collaborate with physicians, RNs, and therapists to deliver patient-centered care.
✔️ Record progress and promptly report changes in patient condition.
✔️ Uphold infection control standards and facility protocols.
✔️ Provide compassionate care to support rehabilitation and recovery goals.

✅ Qualifications
✔️ Education: Graduate of an accredited LPN program.
✔️ Licensure: Active Vermont LPN license and current BLS certification.
✔️ Experience: Minimum 1 year in skilled nursing, rehab, or long-term care.
✔️ Technical Skills: Confident in medication administration, wound care, and EHR documentation.
✔️ Soft Skills: Excellent communication, empathy, and adaptability in high-paced settings.
