6 Home Health APPs Needed in NJ (Home Visits)

Home visits in New Jersey

Start: June

13-week assignment

Location: Home visits across Ocean County (6 providers needed), Middlesex, and Morris County. Please specify your preferred county when submitting your application.


Scheduling:

Must be available between 8 AM – 8 PM (Scheduled for an 8-hour shift within this window, e.g., 8 AM–5 PM or 12 PM–8 PM)

One-hour lunch break included

Schedules are released 2–3 weeks in advance

Full-time, 40 hours/week, Monday through Saturday (flexible within that window)


Job Details:

Conduct 5–6 home health assessments per day

Paid for a full day, regardless of patient volume

Perform annual risk assessments and review of chronic conditions

No treatment or prescribing – assessment-only role

Escalate urgent findings to the patient’s PCP

If applicable, conduct A1C tests only (other screenings left for the patient)

Travel required; mileage reimbursed daily


Equipment:

iPad

Blood pressure cuff

Lab kits for patient use


Credentialing:

1–2 week turnaround

Must have an active NJ license and NJ Medicare number


Qualifications:

Must be licensed in NJ
Mileage will be reimbursed daily
Board-Certified Physician Assistant or NP
