Practice Setting: Outpatient Free-Standing Community Cancer Center
Average Number of Patients Per Day: 16-20
Trauma Level/Designation (Hospital): Level 2
Board Certified - Required
Active IL license - Required at time of name clear
Availability - Please provide at time of name clear - must be available FT
Provider that speaks Spanish - Highly preferred, please note this is the submission, if applicable.
