NP Home Visits in Ocean County

Job description

Location: Home visits across Ocean County (NJ)

Scheduling:
-Must be available to work 8 am- 5 pm or 9 am - 6 pm, 3-4 shifts per week
-One-hour lunch break included
-Schedules are released 2–3 weeks in advance

Job Details:
-Conduct 5–6 home health assessments per day
-Paid for a full day, regardless of patient volume
-Perform annual risk assessments and review of chronic conditions
-No treatment or prescribing – assessment-only role
-Escalate urgent findings to the patient’s PCP
-If applicable, conduct A1C tests only (other screenings left for the patient)

• 8-hr shifts
• No Call
• Part-time available
